摘要
目的:应用犬低温体外循环灌注模型,评价在心脏停跳液中添加三七总皂甙(PNS)的心肌保护作用。方法:实验犬18只,雌雄不分,随机分为3组(每组6只)。1组为对照组,用冷钾停跳液,2组为PNS(50mg/L)小剂量组,3组为PNS(150mg/L)大剂量组。结果:心功能指标恢复百分数,超氧化物歧化酶和脂质过氧化产物丙二醛生成,肌酸激酶及其同工酶活力3组比较:1组缺血再灌注后心功能恢复较差,超氧化物歧化酶明显下降(P<0.01),丙二醛及心肌酶明显升高(P<0.01),2组、3组则变化不明显(P>0.05)。1组与2组、3组比较有显著差异(P<0.01),2、3组间亦有显著差异(P<0.01)。心肌超微结构的保护1组最差,3组最好。结论:在心脏停跳液中加较小剂量的PNS具有良好的心肌保护作用,加大剂量效果更佳。
? Objective:To evaluate the myocardial protective effect of total saponins of Panax notoginsen(PNS,a kind of Chinese herb medicine)with cardioplegic solution in dogs. Methods:Eighteen dogs were divided into three groups(n=6 in each group):Group 1,received cold potassium cardioplegic solution(CPCS);group 2,CPCS+PNS(50 mg/L)and group 3,CPCS+PNS(150 mg/L).The dog heart was subjected to 90 minutes of ischemic arrest,and then reperfused for 30 minutes at 28℃.The percentage of the cardiac functional recovery,superoxide dismutase(SOD) activity,malondialdehyde(MDA) production,creatine kinase(CK) and its isoenzyme (CKMB) release were measured before ischemia,90 minutes of ischemia,and 30 minutes of reperfusion.Ultrastructures of the myocardium were examined at the last two time intervals. Results:The results showed that the percentages of the cardiac functional recovery including mean arterial pressure,left ventricular pressure,etc.were less and SOD activity was decreased (p<001),while MDA production and CK,CKMB release were increased (p<001)in group 1 after reperfusion.In contrast,there were no apparent changes in both group 2 and group 3(p>005).The differences were significant between group 1 and other 2 groups (p<001) and also between the latter 2 groups(p<001).Myocardial ultrastructures were well preserved in group 3,but poorly in group 1. Conclusion:This study indicates that small dose of PNS added to the cardioplegic solution has a good myocardial protective effect.Increasing the dosage of PNS shows even better results.
