腹腔镜肠粘连松解术12例临床分析

Clinical analysis on the laparoscopic enterolysis for 12 cases

目的探讨腹腔镜手术治疗肠粘连的临床效果。方法利用腹腔镜松解术治疗肠粘连12例,用电钩、分离剪分离切断粘连带。结果11例在腹腔镜下顺利完成肠粘连松解术,住院3-7 d后痊愈出院;其中1例手术后1 d出现腹痛、肌紧张等腹膜炎症状,行急诊剖腹探查,见肠管壁有5 mm破裂,行破裂段小肠切除、肠吻合术,住院10 d后痊愈出院。结论腹腔镜松解术治疗肠粘连不仅具有创伤小、恢复快、费用少、痛苦轻、瘢痕小等微创外科的优点,而且术后发生再粘连的机会显著减少,明显优于开腹手术。

Objective To explore the clinical effect of laparoscopic enterolysis. Methods Totally 12 cases underwent laparoseopie enterolysis with electro-claw and clipper mutilating the adhering zone. Results The operations of 11 had been practiced successfully. One with peritonitis symptom such as bellyache and muscle tonus. A5 mm slit was explored by exploratory laparotomy. Small bowel resection and enteroanastomosis were performed then. Conclusion The laparoscopic enterolysis has advantages of minimally invasion, quicker recovery, lower expense, less cicatricle, and so on. The chance of adhere again after operation obviously reduced,which is better than laparotomy.
