彩色多普勒超声诊断急性附睾炎的临床分析

Experiences of color doppler ultrasonography for diagnosing acute epididymitis

目的探讨彩色多普勒超声(CDU)用于诊断急性附睾炎的临床价值。方法回顾性分析87例经CDU诊断的急性附睾炎患者的超声影像资料。结果所有病例超声影像显示均有不同程度的附睾肿大,其中弥漫性肿大67例,局限性肿大20例(头部肿大3例,体部肿大4例,尾部肿大13例)。附睾头部平均厚度1.4 cm,体部0.78 cm,尾部1.5 cm。18例附睾尾部表现为结节样改变,9例为双侧附睾炎,5例合并有睾丸炎。8例超声影像显示有合并症,其中5例合并鞘膜积液,3例伴有同侧精索静脉曲张。87例临床治疗后均确诊为急性附睾炎。结论彩色多普勒超声能较好地显示炎性附睾的血流和形态学变化,诊断准确率高,可作为附睾炎的首选检查方法。

Objective To explore the clinical value of color Doppler ultrasonography （CDU） in the diagnosis of acute epididymitis. Methods Ultrasonographic data of acute epididymitis in 87 patients （96 epididymises） were retrospectively analyzed. Results The ultrasonographic image of 87 patients shew epididymal enlargement ; 67 patients were diffuse enlargement and 20 patients were focal involvement with enlargement of the head （3 cases） ,body （4 cases） or tail （ 13 cases）. The mean thickness of the head was 1.4 cm and those of the body and tail were 0.78 cm or 1.5 cm respectively. The images of 18 patients shew the nodular changes of the miles. The hibateral epididymitis was in 9 patients and 5 patients with orchitis. 5 patients were complicated with hydrocele, 3 patients with homolateral varicocele. The ultrasonographic diagnosis of 87 patients was consistent with that of clinic. Conclusion CDU can better show the morphological and hematological changes of the inflamed epididymis and high accuracy in diagnosis of epididymitis and it is first examination method selected to patients with epididymitis.
