吉西他滨联合铂类药物治疗晚期胸腺癌疗效观察

Gemcitabine combined with cisplatin/carboplatin in the treatment of advanced thymic carcinoma

目的探讨吉西他滨联合铂类药物治疗晚期胸腺癌的疗效以及不良反应。方法入选的15例经病理学或细胞学确诊的晚期胸腺癌患者,采用吉西他滨联合顺铂或卡铂治疗,至少2个疗程以上,根据WHO实体瘤近期疗效评价标准评价疗效,根据美国国立癌症中心(NCI)常见毒性分级标准(3.0版)评估不良反应。结果无完全缓解(CR)病例,3例为部分缓解(PR),8例为稳定(SD),4例为进展(PD),疾病控制率为73.3%(11例/15例)。中位生存时间13月,1年生存率为46.7%。主要不良反应为粒细胞下降、贫血和胃肠道反应。结论吉西他滨联合铂类药物治疗晚期胸腺癌疗效确切,不良反应发生率低,耐受性较好。

Objective To evaluate the efficacy and adverse reactions for chemotherapy with gemcitabine combined with cisplatin/carboplatin in the treatment of advanced thymic carcinoma. Methods Fifteen patients with advanced thymic carcinoma, who had been confirmed with pathology or cytology and failed to previous chemotherapy, were received gemcitabine combined with cisplatin or carboplatin for 2 or more cycles. Results Without complete regression （CR） being observed, partial response （PR） rate was 20.0%, stable disease （SD） was 53.3%, progression of disease （PD） was 26.7%, disease control rate was 73.3%. The median survival time was thirteen months and 1 year survival rate was 46.7%. The common adverse reactions were leucopenia, anemia and gastrointestinal response. Conclusion Gemcitabine combined with cisplatin/carboplatin for advanced thymic carcinoma was effective and feasible. So gemcitabine is one of the choices for thymic carcinoma patients who need two or more line therapy. It is well tolerated with minimal adverse reactions.
