彩色多普勒在乳腺肿块诊断中的应用

Diagnostic Value of Color Doppler on Patients with Mammary Gland Tumor

目的:探讨彩色多普勒在乳腺肿块的诊断价值。方法:选取2007年1月至2008年1月间在我院接受彩色多普勒诊断且均经手术病理证实的乳腺肿块女性患者124例,总结分析其声像特征、诊断准确率等。结果:本组经病理证实良性肿块99例,乳腺癌25例。超声准确诊断107例,诊断符合率为86.29%,误诊率13.71%。本组肿块大小为120mm×90mm×70mm^11mm×13mm×14mm。良性肿块的边界比较清晰,形态规整,可有包膜;恶性肿块则边界形态不规则,皮下组织层次模糊。彩色多普勒表现为良性肿块无血流或可见周边点状血流,恶性肿块可见穿支血流。恶性肿块组在ESV、R I方面明显高于良性肿块组,两者比较差异均具有统计学意义(P<0.05)。结论:彩色多普勒在乳腺肿块的诊断中具有较大的价值,值得应用。

Objective： To investigate the diagnostic values of color doppler on patients with mammary gland tumor. Method： 124 cases with mammary gland tumor confirmed by opration and pathology who received color doppler ultrasonic diagnosis in our hospital from January in 2007 to January in 2008 were selected and the coincidence of ultrasonic diagnosis and feature of acoustic image were analyzed. Result： The 99 cases were innocuousness and 25 breast carcinomas confirmed by operation and pathology. 107 cases were diagnozed correctly by color doppler and the coincidence of ultrasonic diagnosis was 86.29% , and misdiagnosis rate was 13.71%. The mammary gland tumor were 120mm×90mm×70mm-11mm×13mm×14mm The innocuousness tumor were with clear boundary, regular shape, few with peplos, and malignancy tumor were with not clear or regular shape, ambiguity level of subcutaneous organization. The CDFI showed that there were punetiform blood flow signal around the innocuousness tumor and perforating branch were found in malignancy tumor. The malignancy tumor were higher in the level of ESV and RI than that in innocuousness tumor （P 〈 0.05 ）. Conclusion： The diagnosis of color doppler on patients with mammary gland tumor is valuable, so it is worth being utilized widly.
