The Effect of Transurethral Resection of the Prostate Associated with 2 μm Laser Technique Treat Large Volume Benign Prostatic Hyperplasia

摘要 目的:探讨经尿道前列腺电切术(TURP)配合2μm激光联合治疗大体积BPH的价值。方法:对32例BPH患者(前列腺重量均>100 g)随机采用2μm激光与TURP加2μm激光两种手术方法治疗,其中2μm激光治疗14例,TURP加2μm激光治疗18例,对两组疗效进行比较。结果:两组手术前后国际前列腺症状评分(IPSS)、生活质量评分(QOL)、最大尿流率(Q_(max))均较术前显著改善,但差异无统计学意义(P>0.05)。2μm激光组手术时间平均(142.5±32.0)min,TURP加2μm激光组则为(110.2±28.0)min,后者明显短于前者,差异有统计学意义(P<0.05)。两组术中出血量、术后膀胱冲洗时间和导尿管留置时间之间差异无统计学意义(P>0.05)。结论:TURP加2μm激光联合治疗大体积BPH具有与2μm激光治疗相似的疗效,同时能有效减少手术时间,是一种更适合于大体积BPH治疗的手术方式。 Objective.. To investigate the value of treatment for large volume benign prostate hyperplasia (BPH) by combination of transurethral resection of the prostate(TURP) and 2μm laser technique. Methods: 32 cases with BPH (each prostate weighting more than100 gram) were divided into two groups randomly. One group with 14 cases was treated by the 2 μm laser technique method, while the other group with 18 cases was treated by TURP combined with 2 μm laser technique. The therapeutic effects of both ways were compared. Results:International Prostatic Symptom Score (IPSS), Quality of Life (QOL) and Maximum Urine Flow Rate (Qmax) were significantly improved after both of the procedures; and there was no significant difference in the improvement of subjective symptoms and objective signs between those two procedures (P〉0.05). Mean operative time was (142.5±32.0) min for the 2 μm group and (110.2±28.0) min for the TURP+2 μm group. The resection time was shorter for TURP+2 μm group than 2 μwith significant difference (P〈0.05). There was no significant difference for the amount of bleeding during operation, bladder washout time after operation and the time of urinary catheterization in of both groups (P〉0.05). Conclusions:Combination of TURP and 2 μm laser technique has the same therapeutic effect as that of 2μm. In addition, it can effectively shorten the operation time, and it is more suitable for large volume BPH.
