摘要
目的为区域动脉灌注疗法中胰腺动脉的选择提供解剖学依据。方法采用数字减影血管造影技术对临床血管造影检查者22例进行血管造影,观察胰腺动脉分布。结果胰头部血液供应主要依靠胰十二指肠上动脉、胰十二指肠下动脉以及胰十二指肠前后动脉弓。胰颈部的动脉血液供应主要依靠胰十二指肠上动脉左支和胰背动脉右支。胰体、胰尾部动脉血液供应主要依靠胰背动脉、胰大动脉和胰尾动脉。结论区域动脉灌注插管,胰头部病变可选择胃十二指肠动脉或胰十二指肠下动脉;胰颈部位选择胰背动脉;胰体部选择脾动脉或胰大动脉,胰背动脉;胰尾部选择胰大动脉或胰尾动脉。
Objective To provide the anatomical basis for selecting the pancreatic artery in local arterial infusion therapy. Methods The distribution of pancreatic arteries was observed from 22 patients who underwent digital subrtaction angiography (DSA). Results Blood supply in head of pancreas was mainly originated from the superior and inferior pancreaticoduodenal artery, anterior and posterior pancreaticoduodenal arterial arches. Blood supply in neck of pancreas was mainly originated from left branch of superior pancreaticoduodenal artery and right branch of dorsal pancreatic artery. Blood supply in body of pancreas and tail of pancreas was mainly originated from dorsal pancreatic artery, great pancreatic artery and caudal pancreatic artery. Conclusions Gastroduodenal artery and inferior pancreaticoduodenal artery could be chosen to perform a regional transcatheter arterial perfusion when lesion located in the head of pancreas; if the lesion located in the neck of pancreas, dorsal artery could be chosen; splenic artery, great pancreatic artery and dorsal pancreatic artery could be chosen when the lesion located in the body of pancreas; while if the lesion located in the tail of pancreas, great pancreatic artery and pancreatic tail artery could be chosen.
