The clinical value of extending culture of embryos with low morphological scores

摘要 目的探讨对形态学评分较低的胚胎进行体外继续培养的临床价值。方法对低评分胚胎体外继续培养的104个IVF-ET/ICSI周期患者的临床资料进行回顾性分析,观察继续培养后获得冷冻胚胎的个数及这些冷冻胚胎冻融后移植的临床妊娠率。结果 104个IVF-ET/ICSI周期取卵后第三天(D3)低评分胚胎经体外继续培养有32个周期获得冷冻胚胎,冷冻胚胎拥有率30.76%,共培养了606枚低评分胚胎,获87枚冷冻胚胎,冷冻胚胎获得率14.36%(87/606)。23个冻融胚胎移植周期中有3例临床妊娠;103例累计妊娠24例,累计妊娠率23.3%。结论继续培养能有效筛选出具有继续发育潜能的低评分胚胎,提高了胚胎的利用率和累计妊娠率,具有一定的临床价值。 Objective: To explore the value of extending culture of embryos with low morphological scores in the IVF/ICSI treatment. Methods: A total of 606 embryos with low morphological scores were collected from 104 IVF/ICSI cycles and subject to extending culture. The clinical data of the patients who had undergone the extending culture of embryos with low morphological scores were retrospectively analyzed. The count of frozen embryos from extending culture and the clinical pregnancy rate of these frozen-thawed embryo transfer cycles were observed. Results: 32 of 104 IVF/ ICSI cycles (30.76%) had got frozen embryo(s) from extending culture. 87 frozen embryos were yielded from the extending culture of 606 embryos with low morphological scores (14.36%). With these frozen-thawed embryos transferred, 3 additional pregnancies were achieved in 23 cycles, which gave cumulative clinical pregnancy rate of 23.3%. Conclusions: Extending culture can sort out the embryos with development potential from those with low morphological scores, and thus make better use of the embryos and improve the cumulative clinical pregnancy rate.
