急性非静脉曲张性上消化道出血312例诊疗体会

目的总结急性非静脉曲张性上消化道出血的临床诊疗经验。方法对我院近2年收治的312例急性非静脉曲张性上消化道出血患者的病例资料进行回顾性分析。结果急性非静脉曲张性上消化道出血主要病因是消化性溃疡、急性胃粘膜病变和贲门撕裂综合征。大多数病例没有既往胃病史。奥米拉唑序贯治疗治愈率达95%。结论注意急性上消化道出血的少见临床表现有助于减少漏诊。尽早胃镜检查有助于早期确诊。奥米拉唑序贯治疗有助于提高急性上消化道出血的治愈率。

Objective To summarize the clinical experience of acute nonvariceal upper gastrointestinal bleeding（ANVU GIB）. Methods 312 cases ofANVU GIB patients were retrospectively analyzed. Results Major cause of acute non-variceal upper gastrointestinal bleeding were peptic ulcer, acute gastric mucosal lesions and gastric cardia syndrome tear. There was no previous history of stomach trouble in the majority of cases. The cure rate of omeproazole sequential therapy was 95%. Conclusion Attention to the rare clinical manifestations ofANVU GIB contribute to the reduction of missed diagnosis. Gastroscopy as soon as possible contribute to early diagnosis. Omeproazole sequential therapy helps improve the cure rate of acute upper gastrointestinal bleeding.
