摘要 目的探讨P27、CyclinD1和CyclinE在原发性胆囊癌组织中的表达及其临床意义。方法采用免疫组化SP法检测P27、CyclinD1和CyclinE在胆囊癌组织中的表达状况,并与同期手术的胆囊良性肿瘤和胆囊炎性疾病组织作对照。结果①P27在胆囊炎性组织中的高表达阳性率为80%(16/20),在胆囊良性肿瘤组织中为75%(15/20),两者均明显高于其在胆囊癌中的表达[37.3%(28/75),P<0.01];CyclinD1在胆囊癌组织中表达阳性率为58.7%(44/75),显著高于在胆囊良性肿瘤[15%(3/20)]和胆囊炎性疾病[10%(2/20)]中的表达(P<0.01)。Cy-clinE在胆囊癌组织中表达阳性率为60%(45/75),显著高于在胆囊良性肿瘤[15%(3/20)]和胆囊炎性疾病[10%(2/20)]中的表达(P<0.01)。②P27、CyclinD1和CyclinE的表达与胆囊癌病理分级、临床分期有关(P<0.01)并与胆囊癌患者的预后明显相关(P<0.01)。③P27和CyclinD1表达之间呈负相关(r=-0.864,P<0.01),P27和CyclinE表达之间呈负相关(r=-0.685,P<0.01),CyclinD1和CyclinE表达之间呈正相关(r=0.724,P<0.01)。结论P27表达下降,CyclinD1和CyclinE阳性高表达导致细胞周期的调控异常,该机制可能参与了胆囊癌的发生和发展过程。 Objective To explore the expression rate of P27, CyclinD1 and CyclinE protein and their clinical significance in the primary gallbladder carcinoma. Methods The expression of P27, CyclinDl and CyclinE in 75 cases of gallbladder carcinoma were detected with immunohistochemical staining technique (SP method), 20 cases of benign tumor of gallbladder and of cholecystitis tissues were collected as control, respectively. Results (1)The positive expression rate of P27 in cholecystitis was 80% (16/20), that in benign tumor of gallbladder was 75% (15/20), both of them were higher than that in gallbladder carcinoma [37.3%(28/ 75), P〈0.01]. The positive expression rate of CyclinD1 in gallbladder carcinoma was 58.7% (44/75), which was significantly higher than that in benign tumor of gallbladder (15%, 3/20) and in cholecystitis (10%, 2/ 20), P〈0.01. The positive expression rate of CyclinE in gallbladder carcinoma was 60% (45/75), which was significantly higher than that in benign tumor of gallbladder (15%,3/20) and in cholecystitis (10%, 2/20), P〈 0.01. (2)The expression of P27, CyclinDl and CyclinE protein correlated closely to the significant difference of the histological grade, TNM stage (P〈0.01) among the patients of gallbladder cancer significantly related to the prognosis (P〈0.01). (3)There was negative correlation between P27 and CyclinD1 expression (r= -0.864, P〈0.01), negative correlation between P27 and CyclinE expression (r=-0.685, P〈0.01), positive correlation between CyclinD1 and CyclinE expression (r=0.724, P〈0.01). Conclusion The decreased expression of P27, the high expression of CyelinD1 and CyclinE result in abnormal regulation of cell cycle, which may be related to the gallbladder carcinogenesis and progression.
