鼓膜造瘘治疗航空性中耳炎两例并文献复习

Tympanic membrane ostomy on aerotitis media—two cases report with review of the literature

目的观察鼓膜造瘘对航空性中耳炎的治疗效果。方法分析两例航空性中耳炎的临床诊治和随访经过,并复习国内外相关文献。结果于鼓膜造瘘后飞行或低压舱模拟飞行,在上升和下降过程中患者均无不适,经2年以上的随访,患者飞行或常年旅行出差没有再出现术前耳压痛难忍的痛苦症状,中耳无感染,听力没有明显的影响,飞行或乘机飞行时未出现过眩晕。结论鼓膜造瘘是治疗航空性中耳炎的有效方法之一,应加大宣教力度,使更多的军事飞行人员自愿接受此手术,重返蓝天,维护和提高航空兵部队的战斗力。

Objective To sum up effect of tympanic membrane ostomy on aerotitis media. Methods Analyzing the clinical diagnoses, treatments and follow-up of two aircrew who suffered from aerotitis media, as well as reviewing relevant literature. Results After tympanic membrane ostomy, the patients felt well while flying or during an examination taken in the low-pressure cabin which simulated take-off and landing in flight. Following up for two years after tympanic membrane ostomy, they suffered no more unbearable otalgia while piloting or during business travels on regular basis, with no middle ear infections, no obvious hearing problems, and no vertigo in flight. Conclusion As an effective method for curing aerotitis media, tympanic membrane ostomy is so worth widespread publicity that more military aircrew would accept it self-consciously and thus could return to their post soon, which helps maintain and improve the combat capacity of air force division.
