36例皮瓣移植术后高压氧治疗效果观察

The effect of hyperbaric oxygen therapy in 36 cases of skin flap transplantation

目的探讨高压氧(HBO)治疗对皮瓣移植术后存活率的影响。方法对36例皮瓣移植术后患者采取常规+HBO治疗的方法,前三天2次/d,3d后1次/d;共治疗5-35次,平均15.6次。同时观察肿胀渗出消退时间、皮肤红润改变时间以及皮瓣坏死面积等,与常规治疗组对照。结果本组36例,各项观察指标显著少于常规组,其中27例移植皮瓣全部存活,7例基本存活,存活率为94%,明显高于常规组78%,差异有统计学意义(P<0.05)。结论皮瓣移植术后早期行HBO治疗,可提高成活率。

Object＇re To study the effect of survival rate of hyperbaric oxygen （HBO） treatment on skin flap transplantation, Methods 36 patients were treated by hyperbaric oxygen and conventional treatment. Therapy of hyperbaric oxygen was done twice daily at the first three days and once daily after three days. The number of treatment for each hyperbaric oxygen patient was from once to 35 times and the average number was 15.6 times. The time of exudative and swelling disappeared and ruddy skin changed and area of necrotic skin flap were observed. All of these in group of hyperbaric oxygen and conventional treatment were combined with the group of conventional treatment. Results The time of exudative and swelling disappeared and ruddy skin changed was shorter in 36 cases of hyperbaric oxygen than the group of conventional treatment. The area of necrotic skin flap was smaller in group of hyperbaric oxygen than the group of conventional treatment. Survival rate of transplanted skin flap in group of hyperbaric oxygen was 94% and significantly higher than 78% of conventional group. There was statistically significant between two groups （P 〈0.05）. Conclusion Treatment of hyperbaric oxygen has been done early after operation can improve survival rate of transplanted skin flap.
