心、脑血管疾病D二聚体的检测及意义

Determination and significance of D dimer in patients with cardiovascular and cerebrovascular diseases

为了探讨Ｄ二聚体（Ｄｄｉｍｅｒ，ＤＤ）与心、脑血管疾病的关系，采用ＥＬＩＳＡ双抗体夹心法对７２例心血管病人、５１例脑血管病人和７１例健康对照，进行了血浆Ｄ二聚体的测定。在年龄、性别等因素均衡的基础上，显示心、脑血管病例组的Ｄ二聚体含量分别为１１６ｍｇ／Ｌ和０８３ｍｇ／Ｌ，均显著高于对照组（０４３ｍｇ／Ｌ）。进一步将两病例组分为心肌梗塞、非心梗性冠心病、脑梗塞和脑出血亚型后，发现除了脑出血组外，其余三组的ＤＤ水平仍显著高于对照（Ｐ＜００５）。结果揭示Ｄ二聚体的检测在心、脑血管疾病发生中具有重要诊断价值，其含量可作为疗效观察和预后判断的指标之一。此外，根据Ｄ二聚体水平鉴别脑梗塞和脑出血的实际意义值得深入研究。

To find out the relationship between D dimer (DD)and cardiovascular and cerebrovascular diseases,the plasma concentration of D dimer was measured in 72 patients with cardiovascular disease,51 with cerebrovascular disease and 71 health controls by using the sandwith ELISA.On the basis of matching factors in year、 sex etc,the level of DD in cardiovascular and cerebrovascular diseases was 1 16 mg/L and 0 83 mg/L respectively,and was higher than that in controls (0 43 mg/L).Then,the investingation on the myocardial infarction,other coronary heart disease,cerebral infarction and cerebral hemorrhage showed that the DD in preceding three diseases were higher ( P < 0.05 ),while in cerebral haemorrhage did not show significant higher ( P >0.05).The result suggests that the DD detection is valuable for the diagnosis of cardiovascular and cerebrovascular diseases.The level of D dimer could make an important mark of treatment effect for the two kinds of diseases.The differentiation of the subtypes of cerebrovascular diseases according to the level of D dimer needs to make a thorough study.
