高容量血液滤过防治多器官功能障碍中血管内皮细胞功能的变化 被引量:2

High-volume hemofiltration prevents changes of vascular endothelial cell function in pigs with multiple organ dysfunction syndrome

摘要 目的:探讨在高容量血液滤过(HVHF)防治多器官功能障碍综合征(MODS)中血管内皮细胞功能的变化及意义。方法:18只健康雄性家猪在接受失血性休克复苏再灌注联合内毒素血症的两次打击后,随机分为模型组(MODS组,n=9)和血液滤过组(HF组,n=9)。判断MODS的发生,并计算MODS发生率和病死率。利用ELISA法检测两组不同时间点血清中TXB2和6-keto-PGF1α的浓度及可溶性E-选择素(sE-selectin)和可溶性VCAM-1(sVCAM-1)的表达情况。结果:与MODS组比较,HF组MODS发生率和病死率明显降低(P<0.01)。HF组血清中TXB2浓度较MODS组明显降低,6-keto-PGF1α有显著升高(P<0.01);HF组血清中sE-selectin的表达在滤过开始后24h低于MODS组(P<0.01),而在滤过72h后又高于MODS组(P<0.05);同时血清中sVCAM-1的表达HF组在滤过48h后低于MODS组(P<0.05),而处死前又高于MODS组(P<0.01)。结论:在复合打击后早期予以HVHF干预,可有效改善血管内皮细胞功能,从而发挥防治MODS的作用。
