摘要 目的探讨高频彩色多普勒超声在乳腺癌的诊断价值。方法回顾性分析经手术病理确诊的48例乳腺癌患者的高频彩色多普勒超声图像,分析内容包括二维超声形态学指标,肿块内血流信号等级、动脉血流频谱形态。结果典型乳腺癌声像特征为外形不规则,边界不光滑,呈毛刺状或蟹足状,内部回声不均匀,砂粒样钙化,后方回声衰减,组织浸润,纵横径比值>0.77,血流信号等级以Ⅱ级、Ⅲ级为主,动脉血流频谱形态常表现为收缩期峰值前移,上升波和下降波陡直,舒张期始波在中点以下,末期常无血流或出现反向血流频谱。结论高频彩色多普勒超声对乳腺癌有很高的诊断价值,具有良好的应用前景。 [Objective] To approach the diagnosis significance of high-frequency chromatic colour Doppler for mammary adenocarcinoma. [Methods] 48 cases of mammary adenocarcinoma finally diagnosised by pathology were retrospectively analyzed in terms of morphology index of 2D (2-dimensional) ultrasound, the grade of bloodstream wink in turnout, the morphous of frequency spectrum of artery bloodstream. [Results] The typical acoustic image of mammary adenocarcinoma was follows: its shape was irregnlar, its bouncary was not slick, its appearance was like barb or crab foot, internal echo was uneven, calcification was the same as sand, pone echo was attenuated, tissue infiltration, the ratios of length wise and transversal 〉0.77, the main grade of bloodstream wink were Ⅱ and Ⅲ, the morphology of frequency spectrum of artery bloodstream usually displayed peak amplitude of period of contraction antelocation, the wave of upgrading and breakdown were steep, the initial wave of relaxing period was under the midpoint, the end stage usually had no bloodstream or emerged backward bloodstream frequency spectrum. [ Conclusion] The high-frequency chromatic colour Doppler is of good value of diagnosis for mammary adenocarcinoma, and has a satisfactory application perspective.
