患者多次电休克治疗后血压的变化及瑞芬太尼应用的临床观察

摘要 目的探讨无抽搐电休克患者血压的变化及瑞芬太尼的作用。方法将90例无抽搐电休克患者分成3组:Ⅰ组30例,治疗前无高血压;Ⅱ组30例,第1次治疗前无高血压,已多次行无抽搐电休克,再次治疗前血压超出正常范围;Ⅲ组30例,治疗前无高血压。Ⅱ、Ⅲ组加用0.3μg/kg瑞芬太尼。观察基础血压(T0)、电休克前(T1)、诱导后(T2)、电休克治疗后(T3)的SBP、DBP。结果与治疗前比较各组电休克后SBP、DBP显著升高(P<0.01),Ⅱ组与Ⅰ组相比于T1点SBP、DBP有显著差异(P<0.01),于T3点SBP、DBP无显著差异(P>0.05)。Ⅲ组与Ⅰ组比于T3点SBP、DBP有显著差异(P<0.05)。结论多次无抽搐电休克可引起血压升高,0.3μg/kg瑞芬太尼有利于电休克患者血液动力学的稳定和安全。 Objective To investigate blood pressure veiled undergoing electroconvulsive therapies and the use of remifentanil. Methods 90 patients were allocated to three groups(with cases 30 each),control groups and blood pressure increased group. SBP and DBP were observed in all patients based (T0), before ECT (T1) , after anaesthesia (T2) ,and after ECT (T3). Results There were no significant differences in sex, age, weight in three groups. Compared T1 with T3 ,three groups the values of SBP and DBP has a significant difference( P 〈 0.01), Compared with the values of SBP and DBP of I group and blood pressure increased group , there was significant difference at T1 ( P 〈 0.01 )and no difference at T3( P 〉 0.05) , Compared with the values of SBP and DBP of I group and III group , there was significant difference at T3( P 〈 0.05). Conclusion undergoing electroconvudsive therapies can cause blood pressure increase and remifentanil (0.3 μg/kg) had advandages of circulation.
