固尔苏治疗新生儿肺透明膜病45例体会

Experience in treatment of 45 neonatal hyaline membrane disease cases with crossruff

目的观察固尔苏治疗新生儿肺透明膜病(HMD)的效果。方法对45例HMD患儿,分别取仰卧位、右侧卧位和左侧卧位予固尔苏各1/3量缓慢注入气管内,观察用药效果和采取相应的医护措施,并在固尔苏使用前30min及使用后1h、6h、24h、48h取动脉血进行血气分析,于2h及以后2～3d,每天摄胸片观察肺部病变的变化。结果用药后PaO2、PaCO2、pH值及胸片较用药前明显改善,差异有显著性(P<0.05),除3例患儿肺出血死亡,4例患儿为极低出生体重儿且家属经济困难放弃治疗外,38例患儿痊愈出院。结论认为早期、足量应用固尔苏,严密观察病情是保证气管内滴注固尔苏抢救HMD成功的关键。

Objective To investigate the nursing strategy of crossruff treatment in hyaline membrane disease （HMD）. Methods There 45 HMD cases were selected,one third of them were treated slowly by intratracheal injection at dorsaldecubitus,left lateral position and fight lateral position respectively. Then the effect of the treatment with crossruff was observed and the relative nursing strategy was done, arterial blood was collected for blood gas analysis 30 minutes before and lh,6h,24h,48h after treatment. Results The indicators of PaO2, PaCO2, pH of blood gas and the sternum were improved after treatment with crossruff and the significant difference was observed compared with that before treatment （P〈0.05）. Conclusions Crossruff is effective for treatment of HMD cases if used early and with sufficient amount.
