摘要
目的探讨微卡(母牛分枝杆菌菌苗)联合化疗药物治疗耐多药肺结核的临床疗效。方法将240例耐多药肺结核患者按入院先后顺序分为治疗组与对照组,采用化疗方案均为3PaL2AKZThV/15PaL2ZThV。治疗组在使用以上化疗方案的同时深部肌肉注射微卡菌苗,观察2组治疗后肺部病灶吸收情况、痰菌阴转情况、症状改善情况、免疫功能变化情况。结果治疗组与对照组3个月时痰菌阴转率分别为56.8%和26.7%,18个月时为93.3%和73.3%,2组比较均有显著性差异(P均<0.05);2组X线吸收率及空洞缩小、闭合率均有显著性差异(P均<0.05)。结论微卡能调节免疫功能,提高痰菌阴转率,有助于病灶吸收空洞闭合,可作为耐多药肺结核化疗的辅助治疗。
Objective It is to approach the clinical curative effect of microcalory united with chemotherapeutics on tuberculosis of more drugs fast. Methods 240 patients with tuberculosis of more drugs fast were divided into treatment group and control group according to before and behind hospitalization subsequence. The chemotherapy scheme of the two groups applied was both 3PaLaAKZThV/15PaL2ZThV. The treatment group was treated additionally with microcalory deep part intramscular injection at the same time applying above chemotherapy scheme. The bellows focus absorbing circumstance, bacterium in phlegm turning negative circumstance, symptom improving circumstance and immune function changing circumstance in the two groups were observed after treatment. Results The bacterium in phlegm turning negative rates in treatment group and control group were respectively 56.8 % and 26.7 % after treatment for three months and those were respectively 93.3% and 73.3% after treatment for eighteen months. There were both significant differences between the two groups (both P〈0.05). There were all significant differences on the rates of X - ray absorbing, cavity decreased size and cavity closing between the two groups (all P 〈 0.05). Conclusion Microcalory can regulate immune function, heightening the bacterium in phlegm turning negative rate and conduce to focus absorbing and cavity closing. Microcalory can be taken as adjunctive therapy of chemotherapy on tuberculosis of more drugs fast.
