截瘫并肾功能不全患者重度压疮的治疗及护理

Treatment and nursing for paraplegia and renal insufficiency patients with severe pressure sore

目的:探讨截瘫并肾功能不全患者重度压疮的治疗及护理方法。方法:对截瘫并肾功能不全重度压疮患者进行抗感染、营养支持、膀胱造瘘、护肾、血液透析、对症支持等治疗,并进行常规护理和血液透析,剪去压疮坏死组织,清洗消毒创面后,按白糖:复方新诺明(粉末)=5∶1的比例配制混合均匀后导入基底创面内,并包裹固定,根据创面敷料渗液情况换药。结果:治疗1周后见压疮鲜红肉芽生长,65d压疮愈合。结论:复方新诺明配伍白糖治疗截瘫并肾功能不全重度压疮患者,具有消炎、高渗及营养的协同作用,可促进新鲜肉芽生长和创面的愈合速度,同时给予相应的护理,创面愈合较好。

Objective： To investigate treatment and nursing for paraplegia and renal insufficiency patients with severe pressure sore. Methods： Except for routine nursing and hemodialysis, anti - infection, nutrition support, bladder stoma, kidney restoration, hemodialysis, and symptomatic treatment support were employed to patients. After cutting necrotic tissue, cleansing and disinfection of the wound, applied mixture of sugar and cotrimoxazole at the ratio of 5：1 on the basement of wound, and made a wrapped fixation. Changed dressing according to the situation of wound. Results： 1 week later after the treatment, scarlet granulation occurred, and 65d later the pressure sores were healed. Conclusion ： With anti - inflammation, hypertonic and nutritional synergy, the combined treatment with sugar and cotrimoxazole can promote the growth of new granulation and wound healing rate, and it works more effective with the corresponding nursing.
