胎儿单脐动脉的超声诊断及其临床价值(附24例分析)

Clinical value and prenatal diagnosis using ultrasonography of single umbilical artery

目的探讨胎儿单脐动脉畸形产前超声声像图特征及其临床意义。方法回顾性分析2007年11月至2008年12月我院产前超声检查发现的24例胎儿单脐动脉的临床和超声资料。结果24例超声均诊断为胎儿单脐动脉,超声显示在胎儿盆腔膀胱及脐轮斜切面仅见膀胱一侧显示脐动脉走行。其中合并胎儿其他系统发育异常8例;继续妊娠的16例中,有4例在孕程和产程中出现各种异常,余12例胎儿妊娠进程顺利,随访无明显异常发现。结论彩色多普勒超声检查能够准确诊断胎儿单脐动脉,超声检查发现胎儿单脐动脉时需多角度动态观察以明确是否合并其他器官的发育畸形,对于继续妊娠者需密切注意胎儿发育情况和产程。

Objective To evaluate the clinical role and the ultrasonographic features in prenatal diagnosis of single umbilical artery （SUA）. Methods To review 24 fetuses with SUA found in our hospital from November 2007 to December 2008. Their clinical manifestation and pregnancy outcome were followed up. Results In 24 cases, only one umbilical artery could be detected by ultrasonography. Among them, eight cases combinated other abnormalities. In the cases going on with their preg nancy, four cases had disorders during the pregnancy course or the labor, the other 12 cases were fine and there were no abnor-mality in the following-up. Conclusion Ultrasound-Doppler can diagnose the USA correctly. When we find a fetus with SUA, we should analyze it from different aspect to except other organic malformations abnormality. To the cases going on with their pregnancy, we must pay more attention to the fetuses growth and delivery.
