摘要
目的探讨创伤性膈疝的临床特点、诊断及手术治疗方法。方法回顾性分析1996年6月～2008年8月期间45例创伤性膈疝的临床资料,其中开放性损伤22例,闭合性损伤23例;左侧膈疝40例(88.9%),右侧膈疝5例(11.1%);41例伴有合并伤;45例均行手术治疗。结果治愈44例(97.8%),死亡1例(2.2%)。39例随访,无膈疝复发。结论早期诊断和及时手术、正确选择处理顺序和修补方式,是救治成功的关键。
Objective To investigate the diagnosis and therapy of traumatic diaphragmatic hernia （TDH）. Methods The clinical data of 45 patients with TDH were analyzed retrospectively. Of 45 cases, there were 23 cases were of blunt injuries, and 22 cases of penetrating injuries. Forty cases （88.9%） had hernia of left diaphragm, and 5 cases （11.1%） of right diaphragrrL Forty-one cases had combined injuries. Operations were performed on all patients. Results Forty-four eases （97. 8%） were cured, and 1 cases （2. 2%） died due to hemorrhagic shock. During the follow-up of 39 cases, there was no recurrence of diaphragmatic hernia. Conclusion Early diagnosis and timely surgical treatment, correct choice of procedures and repair modesy are the key to improve the curative rate and reduce mortality of patients with TDH.
