幽门螺杆菌根除对儿童急性特发性血小板减少性紫癜的疗效

Effect of Helicobacter pylori eradication on childhood acute idiopathic thrombocytopenic purpura

目的观察幽门螺杆菌(Hp)根除治疗对常规初治的轻度儿童急性特发性血小板减少性紫癜(ITP)的疗效。方法血清Hp阳性的急性ITP初治患儿93例被随机分为治疗组(A组51例)和对照组(B组42例)。B组行常规激素治疗,A组在此基础上加根除Hp治疗。结果A组Hp清除率87.5%;两组间近期疗效无显著性差异,但1年内复发率A组低于B组(P=0.033)。结论Hp清除对儿童急性ITP的疗效无影响,但能减少复发率,对于Hp感染的ITP患儿建议行根除Hp治疗。

Objective To evaluate the effect of Helicobacter pylor （Hp） eradication in children with acute idiopathic thrombocytopenic purpura （ITP）. Methods Ninety-three children with acute ITP and Hp infection were divided into two groups and treated with prednisone and IIp eradication （group A, 51cases） or with prednisone without lip eradication （group B, 42 cases）. Results The Hp eradiation rate was 94.1% in group A. No difference was found in the therapeutic effects on IPT between the two groups, but the recurrence rate in one year in group A was significantly lower than that in group B. Conclusions Hp eradication does not obviously enhance the therapeutic effect on childhood acute ITP, but can decrease the relapse rate in one year. HP eradication therapy is recommended in children with acute ITP and Hp infection.
