带种植体的自体骨块移植纠正种植体植入位置不良初探

Application of Segmental Osteometomy in Dental Implant Reposition:Two Case Reports

目的评价带种植体自体骨移植纠正种植体三维位置不良的临床效果。方法对2例患者2颗由于植入位置不良弃用或者修复美学效果不良的种植体,采用空心环钻带骨取出后,重新制备种植窝,植入带种植体的柱状骨块,同时采用引导骨组织再生技术,6个月后行2期修复。结果2颗移植后的种植体形成骨整合,修复后美学效果改善明显。结论采用带种植体自体骨非血管化移植是纠正种植体三维位置不良,改善修复美学效果的一种可供选择的方案。

Objective To investigate the clinical use of segmental osteometomy in reposition of the implant to a more favorable orofacial position. Methods 2 patients with 2 misplaced implants were included. The trephine retrieval （diameter 6 mm） was used to vertically cut through the ridge around the implants. The implants was replaced in an ideal three-dimensioned position, Bone filler （Bio-osstem） was gaffed into the defect and the facial aspect, and the recordablc collagen membrane was trimmed and adapted to cover the defect of the facial aspect. 6 months after surgery, a healing a- butment was inserted to optimize the soft tissue profile. Results Osseointegration was observed in both cases. The facial contour of 2 implants was greatly improved, and the esthetics around the implants were enhanced. Conclusion Segmen- tal osteometomy to reposition the implant in a more favorable orofacial position can equally correct the malposition, get osseointegation.
