血管活性药物对盐酸吸入性肺损伤大鼠动脉血氧分压的影响

摘要 目的研究血管活性药物对急性肺损伤(ALI)大鼠动脉血氧分压(PaO2)的影响。方法28只SD大鼠随机均分为四组,分别静注生理盐水(A组)、艾司洛尔加硝酸甘油(B组)、艾司洛尔加去氧肾上腺素(C组)和异丙肾上腺素(D组)。然后均经气管导管内注入0.1N盐酸1ml/kg造成ALI。记录静脉注入实验药物即刻(T0)、气管注入盐酸后10min(T1)和30min(T2)的PaO2、MAP、HR值。结果各组T1和T2时的PaO2均较T0时明显下降(P<0.01)。C组T1和T2时的PaO2明显低于其他组(P<0.01),而D组PaO2的下降幅度最小,显著高于其他各组(P<0.01)。结论异丙肾上腺素能减轻ALI,合用艾司洛尔和去氧肾上腺素则恶化ALI。 Objective To investigate the effect of vasoactive agents on acute lung injury (ALI) induced by hydrochloric acid in rats. Methods Twenty-eight male SD rats were randomized equally into four groups of saline (group A), esmolol combined with nitroglycerin (group B), esmolol combined with phenylephrine (group C) and isoprenaline (group D). After anesthesia induction and tracheal intubation, 0.1 N hydrochloric acid (1 ml/kg) was instilled into the lungs via endotraeheal tube following intravenous injection with saline or vasoactive agents. Arterial blood gas analysis was performed immediately before hydrochloric acid instillation(T0 ) and at 10 rain(T1 ), 30 rain(T2 ) after that. Results Compared with that at To, arterial oxygen pressure (PaO2) was significantly decreased in the four groups at T1 and T2 (P〈0. 01),which was lower in group C than that in the other three groups(P〈0. 01). However, PaO2 of group D was significantly higher than that of other three groups at T1 and T2 (P〈0. 01). Conclusion ALI could be attenuated by preinjection of isoprenaline, but deteriorated by preinjection of esmolol combined with phenylephrine.
