甲状腺手术中喉返神经的显露

The value of exposure recurrent laryngeal nerve in thyroidectomy

目的探讨甲状腺手术中显露喉返神经(RLN)对预防神经损伤的意义。方法对1998年1月至2008年11月术中显露与未显露RLN的597例甲状腺手术进行回顾性分析。结果共实施597例甲状腺手术,显露组299例,解剖显露RLN391条,喉不返神经1条,损伤RLN3条,损伤率0.77%;未显露组298例,损伤RLN19条,其中永久性损伤3条,损伤率6.4%;两组损伤率经统计学处理差异有统计学意义(P<0.05)。结论甲状腺手术中显露RLN可有效预防神经损伤。

Objective To study the value of exposure recurrent laryngeal nerve （RLN） in the prevention of neural injury in thyroid surgery. Methods The data of 597 cases with exposure RLN and nonexposure RLN in thyroidectomy from January 1998 to November 2008 were analyzed retrospectively. Results There were 597 cases of thyroidectomy operated. In REN exposure group （299 cases） ,there were 391 exposed RLNs, 1 nonrecurrent laryngeal nerve（NRLN） ,3 injuried RLNs, The injury rate of RLN was 0.77%. In RLN non - exposure group （298 cases） ,there were 19 injuried RLNs,3 permanent palsy recurrent laryngeal nerves,the injury rate of RLN was 6.4%. The difference between the two groups were statistically significant（ P 〈 0.05 ）. Conclusion The injury of nerve can be reduced with exposal RLN during the thyroid surgery .
