羊膜移植治疗急性碱烧伤TGF-β1表达的实验研究

摘要 目的探讨角膜碱烧伤后羊膜移植的可行性,并分析TGF-β1在碱烧伤中的作用机制。方法45只健康家兔,以1mol/L的氢氧化钠烧伤右眼制成眼前段碱烧伤模型。随机分为3组,每组15只。Ⅰ组行新鲜羊膜的移植术、Ⅱ组行保存羊膜的移植术、Ⅲ组未行手术作为对照。各组于术后1、2和4周分别处死动物,收集角膜标本做TGF-β1免疫组化检查。结果①在裂隙灯显微镜下观察:羊膜移植组新生血管范围、炎症反应、疤痕化在术后7、14和28d明显低于对照组,新鲜组比保存组好。②碱烧伤后TGF-β1在3组角膜基质均显著表达。羊膜组在各个时间点表达低于对照组(P<0.05),新鲜组比保存组低。在同一组内各个时间点TGF-β1表达无明显差异(P>0.05)。结论羊膜移植可以明显减少碱烧伤角膜TGF-β1的表达及可以有效的减少疤痕的形成;TGF-β1的表达与疤痕增生程度成正相关性,其在疤痕的形成中可能起了重要作用;新鲜羊膜在抑制疤痕的生成方面优于保存羊膜。 [Objectives] To assess the effect of amniotic membrance (AMT), and to investigate the mechanisms of TGF-β1 on corneal alkali injury. [Method] Comeal alkali-bum model of 45 rabbits were established. The rabbit were divided into fresh amniotic transplantation group (fAMT), preserved amniotic transplantation group (pAMT) and control group (C): on the 7th day, 14th day and 28th day after operation. The expression of TGF-β1 in cornea were measured by immunohistoche mical methods. [Results] The contents of PMN, CNV and keloid in the fAMT group were lower than those in the pAMT and both were lower than those in C group. Remarkable differences of expression of TGF-β1 could be observed between the comparison group. The fAMT group were lower in the pAMT groups and both were lower than the C group. But at the same group in different time, the expression of TGF-β1 was no major difference. [Conclusions] Amniotic membrance transplantation can decrease keloid by inhibiting the expression of TGF-β1. Fresh amniotie membrance transplantation has more action on restraining keloid than preserved amniotie membrance transplantation. TGF- β1 has significance on sears, the expression of TGF-β1 has a relation with the scars.
