心电编辑及多时相重建对提高64层螺旋CT冠脉图像质量的临床应用研究 被引量:9

摘要 目的应用心电编辑及多时相重建对提高64层螺旋CT冠脉图像质量的临床应用及研究。方法对150例做冠状动脉CTA的患者进行冠状动脉成像,通过心电编辑、多时相重建方法使心律不齐、呼吸等伪影进行调整使之获得满意的图像。结果150例中,20例由于心率变化出现伪影,后通过心电编辑获得满意图像评分3-4分,占13%,15例呼吸伪影出现错层通过多时相原始数据重建选择最佳时相获得最佳图像,评分3-4分,占1%;10例是患者本身冠状动脉硬化斑块、冠脉支架及体外搭桥等病变影响图像质量,通过时相反复间隔重建选择获得好的图像评分3分,占6.6%;2例由于患者肥胖致使管电流不足出现信噪比下降使图像显示不满意,评分2分;3例患者由于严重心律不齐图像显示不良,评分2-3分,占2%;剩余的100例均通过常规扫描时相获得满意图像,评分3-4分,占66%。结论64层螺旋CT冠状动脉成像具有较高的诊断准确性,但要获得质量好的图像受多种因素的影响,其中心率及呼吸的变化是影响图像质量的主要原因,如何在心脏跳动情况下采集到优质的图像,除要有好的设备及患者、医生、技师通力配合外,通过心电编辑、多时相重建等一系列措施后,最终才能得到一张满意的图像,为广大的冠心病患者服务。本研究具有科学性、先进性、广泛性及实用性。 Objective To investigate the value of electrocardio-edit and multidate reconstruction for 64 SCTA of coronary artery. Methods Analyzed 150 cases of coronary artery CTA. Adjusted artifact ofarrhythmia and breath by electrocardio-edit and multi temporal image. Achiveed satisfactory image. Rusults To appear the artifact 20 cases for change of heart rate.To achive satisfactory image by electrocardio-edit, scored 3-4,13% rate.To appear fault in ply 15 cases for artifact of breath.To achive satisfactory image by selecting the best time phase, score3-4,1% rate. 10 cases for atheroscterolic plaque and PTCA Stenting and off-Pump CABG and so on.To achive the best image by multidate reconstruction interval,score 3,6.6% rate.Two cases were unsatisfactory because of fatness,score 2.Three cases were unsatisfactory because of arrhythmia, score 2-3,2% rate.One hundred cases were satisfactory by common time phase,score 3-4,66% rate. Conclusion The 64 SCTA of coronary artery has superior veracity of diagnosis,but it is susceptibled for variety of factor.Teart rate and breath are primary.To collect superior image not only good equipment and cooperation of patient and doctor and technician but also electrocardio-edit and multidate reconstruction.It is scientific and advanced and extensive and practical.
