摘要
目的探讨高危型人乳头状瘤病毒感染与卵巢癌预后的关系。方法采用原位杂交法检测卵巢上皮性良性肿瘤32例、交界性肿瘤25例、卵巢癌45例中HPV16/18病毒,免疫组化SP法检测血管内皮生长因子(VEGF)、ki-67的表达;用正常卵巢组织作对照。结果(1)卵巢癌中HPV16/18的感染与正常卵巢组织、上皮性良性肿瘤比较均有显著性差异(P<0.001),与交界性肿瘤比较无差异;VEGF、ki-67在卵巢良性瘤与正常卵巢组织间无显著性差异,其余各组间比较均有显著性差异(P<0.05)。(2)低分化卵巢癌中HPV16/18的感染率高(P<0.05),与患者组织学类型及临床分期等无关;分化差、临床分期晚的卵巢癌中VEGF、ki-67高表达(P<0.05),与组织学类型无关。(3)卵巢癌中VEGF、ki-67与HPV16/18感染呈正相关。结论高危型HPV的感染可能与卵巢癌的发生有关,VEGF、ki-67可能协同HPV16/18参与卵巢肿瘤的演进过程。
Objective To explore the relation ship between the high - risk human papillomavirus(HPV) and prognosis of ovarian carcinomas. Methods In situ hybridization technique was used to examine HPV16/18 DNA in 32 cases of benign ovarian epithelial tumors,25 cases of borderline tumors and 45 cases of ovarian carcinoma. Immunohistoehemistry was used to examine the expression of VEGF and ki - 67 protein, which compared to usual ovarian tissues. Results ( 1 ) The positive rate of HPV16/18 was significantly different between ovarian carcinoma and normal epithelial ovarian tissues or benign epithelial ovarian tumors ( P 〈 0. 001 ) , but no significant difference between ovarian carcinomas and borderline tumors. The positive rate of VEGF and ki - 67 was significantly different between the others groups of tissues (P 〈 0.05 ) , except between normal epithelial ovarian tissues and the benign epithelial ovarian tumors. (2)The percentage of positive expression of HPV16/18 was evidently higher in the poorly differentiated and advanced stage ovarian carcinomas than that in the well differentiated group ( P 〈0.05 ) , and was not associated with tumor types or clinical stages. The positive rate of VEGF and ki -67 was evidently higher in the poorly differentiated ovarian carcinomas than that in the well differentiated and early stage group (P 〈 0.05) , and was not correlated with tumor types. (3) The expression of VEGF and ki - 67 was positively correlated with the infeetion of HPV16/18 in ovarian carcinomas. Conclnsion The infection of High - risk HPV may be related to the development of ovarian carcinoma. VEGF and ki - 67 may cooperate with High - risk HPV to advance the progression of ovarian tumors.
