髂腹部皮瓣在深度烧伤创面的应用

Application of lower-abdominal flaps in treating deep burn wounds

目的应用髂腹部皮瓣修复深度烧伤创面。方法用以旋髂浅血管为蒂的腹股沟皮瓣及以腹壁浅血管为蒂的下腹壁皮瓣修复深度烧伤创面38例。结果本组38例中,36例术后创面一期愈合,效果满意;1例瓣下积脓,经引流、植皮后愈合;1例皮瓣部分撕脱,未影响皮瓣成活及创面愈合。结论髂腹部皮瓣解剖恒定,操作较简单,血供良好,是修复手、腕及其下腹部临近IV度烧伤创面的理想方法。

Objective To investigate the application of lower-abdominal flaps in the treatment of deep bum wounds. Methods Fold inguen and lower abdominal flaps were used to treat 38casess of deep burn wounds, with superficial circumflex iliae vessels and superficial abdominal wall blood vessels as the pedicles. Results Thirty-six cases obtained primary intention healing and satisfactory effect; one case reached healing after drainage and skin grafting on account of empyema under flap; one flap shown partly avulsion, but did not effect flap survival and raw surface healing. Conclusion Using lower-abdominal flaps to repair IV burn wounds near hand , carpus and bythus is an ideal method.
