血清MMP-3水平与ACS冠状动脉病变程度相关性研究 被引量:4

Correlation between the serum level of matrix metalloproteinases-3 and severity of coronary lesions in acute coronary syndrome patients

摘要 目的检测急性冠状动脉综合征(acute coronary syndrome,ACS)患者血清基质金属蛋白酶-3(matrixmetalloproteinases-3,MMP-3)水平并与冠状动脉病变程度作相关性分析。方法确诊ACS的135例患者和71例非ACS者均进行冠状动脉造影检查。根据冠状动脉病变程度进行Gensini评分(gensini score,GS)。采用ELISA方法测定血清MMP-3水平。结果ACS组患者不稳定型心绞痛(unstable angina pectoris,UA)组46例,MMP-3浓度(36.66±13.23)μg/L;非ST段抬高型心肌梗死(non-ST-segment elevation myocardial infarction,NSTEMI)组47例,MMP-3浓度(41.82±13.86)μg/L;ST段抬高型心肌梗死(ST-segment elevation myocardial infarction,STEMI)组42例,MMP-3浓度(69.27±34.02)μg/L。NSTEMI组及STEMI组的MMP-3浓度与对照组MMP-3浓度(35.29±13.10)μg/L有显著差异(P<0.01)。将所有ACS组Gensini评分资料根据MMP-3血清浓度的25%、50%、75%分界点分为4组,发现ACS患者MMP-3血清浓度越高,冠状动脉Gensini评分越高。相关分析显示桡动脉血MMP-3血清浓度与冠状动脉Gensini评分明显相关,相关系数0.749,(P<0.01)。结论MMP-3血清水平与ACS冠状动脉血管病变程度存在正相关性,血清MMP-3是ACS的有效生物标志物。 Objective To investigate the relationship between the serum level of matrix metalloproteinases- 3 (MMP-3) and the severity of coronary lesions in acute coronary syndrome (ACS) patients. Methods One hundred and thirty five patients with ACS and seventy one patients without ACS were included in this study, and coronary angiography was performed for all these patients. Whereas Gensini score ( GS ) was used to evaluate the severity of coronary lesions, the serum MMP-3 level in these patients was measured using ELISA. Results Of all ACS patients, 46 cases had unstable angina (UA) with MMP-3 at ( 36.66 ± 13.23 ) μg/L, 47 cases had non-ST-elevation myocardial infarction (NSTEMI) with MMP-3 at (41.82 ± 13.86) μg/L, and 42 cases had ST-elevation myocardial infarction (STEMI) with MMP-3 at (69.27 ± 34.02 ) μg/L. MMP-3 levels in STEMI group and NSTEMI group were significantly different with that in the controls (P 〈 0.01 ). All the patients were assigned to one of four groups according to MMP-3 level with 25, 50, and 75 percentile as cut-off points. It was found that high MMP-3 level was associated with the scores of Gensini in ACS group. Correlation analysis showed that MMP-3 level was positively correlated with the Gensini scores (r = 0. 749, P 〈 0. 01 ). Conclusion The serum MMP-3 level in ACS patients has positive relationship with the severity of coronary artery lesions, hence is an effective biomarker for ACS.
