血清脂联素水平与肥胖、胰岛素抵抗的关系探讨 被引量:10

Relationship among serum adiponectin level,obesity and insulin resistance

摘要 目的探讨血清脂联素水平与胰岛素抵抗之间的关系,以及肥胖和胰岛素抵抗的交互作用对脂联素的影响。方法将167例研究对象分成胰岛素抵抗伴肥胖、单纯胰岛素抵抗、单纯肥胖和正常状态四组,运用方差分析、协方差分析对观察指标进行研究。结果在扣除性别、收缩压、舒张压、甘油三酯和高密度脂蛋白胆固醇对血清脂联素水平的线性影响后,脂联素在四组中的修正均数依次为3.477μg/mL、3.681μg/mL、4.447μg/mL和4.614μg/mL(P=0.033),胰岛素抵抗伴肥胖组的血清脂联素水平明显低于单纯肥胖组(P=0.036)和正常组(P=0.009),未发现胰岛素抵抗和肥胖的交互作用的存在。结论血清脂联素水平降低与胰岛素抵抗之间互为因果,肥胖和胰岛素抵抗均与脂联素水平下降独立相关。 Objective To investigate the relationship between serum adiponectin and insulin resistance and to explore the effects of obesity and insulin resistance interaction on the adiponectin level. Methods 176 subjects were divided into 4 groups: the insulin resistance with obesity, the insulin resistance, the obesity and the control groups. Variance and eovarianee analyses were used in this study. Results After rnling out the linear interference with adiponectin from gender, systolic blood pressure, diastolic blood pressure, triglycerides and high-density lipeprotein cholesterol, the modified means of serum adiponectin levels in the four groups were 3.477μg/mL, 3.681μg/mL, 4.447 pg/mL and 4. 614 μg/mL( P = 0. 033), respectively. Adiponectin in the insulin resistance with obesity group was significantly lower than that in the obesity group ( P = 0. 036) and the control group ( P = 0.009). Conclusions The relationship between serum reduced adiponectin and insulin resistance may be bi-directional. Both obesity and insulin resistance are independently correlated with serum adiponectin.
