记忆合金三维内固定系统在髋臼骨折中的应用

Application of acetabular tridimensional memory fixation system to treat acetabular fractures

目的探讨记忆合金三维内固定系统在髋臼骨折中的应用效果。方法对25例髋臼骨折患者采用手术切开记忆合金三维内固定系统固定,术后随访6~28个月。结果25例患者中术中直视下骨折解剖复位22例（88%）,满意复位2例（8%）,不满意复位（移位4 m）1例（4%）。术后摄X线片复查未发现骨折有再移位。22例于术后1.6个月达骨性愈合;术后2.5个月,患侧髋关节功能达到健侧水平。术后并发异位骨化、关节失用1例,骨化性肌炎致功能障碍1例,感染但未遗功能障碍1例。按照美国骨科学会拟定的疗效评定标准,优18例、良4例、可2例、差1例,优良率88%。结论记忆合金三维内固定系统为髋臼骨折的解剖重建提供了新的有效方法。

Objective To observe the effects of acetabular tridimensional memory fixation system（ATMFS） in treatment of acetabular fractures. Methods 25 cases with acetabular fractures were treated with ATMFS. All patients were followed up for 6 - 28 months. Results During operation,22 patients （ 88% ） were anatomic reduction, 2 patients （ 8% ） were good reduction, and 1 patient（4% ） was not satisfied reduction （displacing 4 mm）. X-ray results showed no fracture displacing after operation. 22 cases got bony union in 1.6 months. Hip joint function of affected side achieve the level of uninjured side after operation in 2.5 months. One case was heterotopic ossification and articular apraxia after operation, myositis ossificans caused functional disorder in one case, infection but no functional disorder in one case. According to the American Academy of Orthopaedic Surgeons formulated clinical grading criterion,excellent in 18 cases,good in 4 cases ,fair in 2 cases and bad in one cases,the good rating was 88%. Conclusion ATMFS is a new and effective method,which can achieve anatomic reconstruction of acetabular fractures.
