左旋肉碱对腹透贫血患者血常规结果的影响 被引量:1

Effects of L-Carnitine on Blood Routine in Peritoneal Dialysis Patients with Anemia

摘要 目的观察左旋肉碱对慢性肾衰(CRF)腹透贫血患者血常规结果的影响。方法将血红蛋白(HB)<90g/L,接受r-HuEPO治疗的16例腹透病人随机分为A、B两组;将HB>90 g/L,不接受r-HuEPO治疗的12例腹透病人,随机分为C、D两组。A、C两组给予左旋肉碱治疗,B、C两组给予等容积生理盐水作为对照组。实验开始前与12周后分别测定血常规及血浆游离肉碱浓度。结果应用左旋肉碱治疗的A、C两组患者的RBC、HB、HCT值均显著高于治疗前(P<0.05),血浆游离左旋肉碱浓度明显高于治疗前(P<0.05),而血常规其他指标无显著性改变(P>0.05)。治疗后A、C两组的RBC、HB、HCT值分别显著高于B、D两组(P<0.05),血浆游离左旋肉碱浓度分别显著高于对照组(P<0.05),而血常规其他指标无显著性改变(P>0.05)。结论左旋肉碱能显著提高腹透贫血患者的RBC、HB、HCT值,而对血常规其他指标无显著性改变。 Objective To observe blood routine changes of L-carnitine on chronic peritoneal dialysis patients with anemia. Methods 16 cases of peritoneal dialysis (Hb 〈 90 g/L)that were given r-HuEPO treatment were randomly divided into group A, group B. 12 cases of peritoneal dialysis (Hb 〉 90 g/L)that were not given r-HuEPO treatment were randomly divided into group C, group D. Group A and group C were given L-carnitine treatment. Group B and group C were given equal volume of normal saline as a control group. Free carnitine concentrations of blood and plasma were measured at the beginning of the experiment and after 12 weeks. Results RBC, HB, HCT and plasma free L-camitine concentration of L-carnitine treatment patients in group A and C with were significantly higher than that before treatment(P 〈 0. 05 ), and other indicators of blood routine were no significant changes(P 〉 0. 05). RBC, HB, HCT of group A and C were significantly higher than those of group B and D after treatment(P 〈 0.05). The plasma free L-carnitine concentrations of group A and C were significantly higher than that of control group ( P 〈 0.05 ), while the blood of other indicators no significant changes(P 〉 0.05). Conclusion L-carnitine can improve RBC, HB, HCT value in peritoneal dialysis patients with anemia, and other indicators of blood have no significant changes.
