糖尿病合并肺结核95例影像学分析

Imaging Analysis of 95 Cases of Diabetes Mellitus Complicated with Pulmonary Tuberculosis

目的探讨糖尿病并发肺结核的影像学特点。方法对95例糖尿病并发肺结核患者进行影像学(X线胸片或CT扫描)检查,同时收集临床资料,分析病灶的特点。结果95例糖尿病并发肺结核患者的病灶表现多为双上肺野分布的多叶、多段的斑片状、大片状阴影和空洞性改变,或表现为急性粟粒性肺结核,单一病灶内多发虫噬状空洞,支气管播散多见。血糖控制后,病灶大多好转。结论糖尿病合并肺结核具有一定的影像学特征:以炎性渗出、干酪变质为主,空洞发生率高,高血糖状态对结核病灶有一定程度的影响。

[ Objective ] To investigate the imaging features of diabetes mellitus complicated with pulmonary tuberculosis. [ Methods]95 patients with diabetes mellitus complicated with pulmonary tuberculosis accepted imaging examination （Chest X-ray or CT scan ）, and clinical data were collected simultaneously. The characteristics of lesions were analyzed. [ Results] Among 95 patients, the morphological distribution features of the pulmonary tuberculosis lesions were mostly multiple lobes and multi-segment large flake shadow or cavitations in bilateral upper lobe of lung, or some presented as acute miliary pulmonary tuberculosis with multiple motheaten pattern cavitations in a single lesions, and bronchus dissemination were usually found. With blood glucose controlled well, the lesions were mostly improvement. [ Conclusion ] The imaging features of diabetes mellitus complicated with pulmonary tuberculosis showed that the lesions were mainly inflammatory effusion and classification with high incidence of cavitations. The hyperglycemia state had some influence on pulmonary tuberculosis lesions.
