摘要
目的对先天性心血管畸形患儿进行电影MRI检查,并与心血管造影(CAG)对照分析,评价无创性电影MRI诊断小儿先天性心血管畸形的价值。方法对31例(3个月至11岁,平均年龄6.5岁)不同类型先天性心血管畸形患儿常规行SE序列成像,然后从SET1WI中选择最佳层面做电影MRI,并与选择性心血管造影结果进行对照研究。结果电影MRI对31例先天性心血管畸形的主要病变诊断与CAG检查结果基本一致,其中25例经手术证实。结论电影MRI对小儿先天性心血管畸形有很高的临床应用价值,能根据诊断需要多方位、多序列成像,能良好地显示心内畸形病变,并能在同一层面上动态显示大血管畸形全貌及血液动力学状况。
Purpose To study children's congenital anomalies of heart and great vessels with cine MRI and to evaluate its diagnostic value by comparison with conventional angiocardiography. Methods Routine SE sequence was performed in 31 cases (3 months to 11 years old, average 6.5 years ) with different varieties of congenital cardiovascular anomalies. The best section of T1WI was selected and used for cine MRI study. All cases had selective angiocardiography for a comparative study. Results The diagnosis of the main pathologies in 31 congenital cardiovascular anomalies were in accordance with those demonstrated by selective angiocardiography. 25 out of 31 cases were verified by operation. Conclusion Cine MRI is highly valuable in the diagnosis of congenital cardiovascular anomalies in children by clearly demonstrating the intracardiac pathologies taking advantage of multiplanar capabilities and multiple sequences. The full display of morphology and hemodynamic changes of anomalous great vessels can be shown on one slice, providing useful diagnostic informations as demonstrated in this study.
