Clinical value of 18F-FDG PET/CT in detecting the recurrence and metastasis of ovarian cancer

摘要 目的:探讨PET/CT在监测卵巢癌术后复发、转移中的应用价值。方法:回顾性分析20例卵巢癌术后患者全身18F-FDGPET/CT显像结果,临床随访时间6~18个月。确诊依据为手术病理检查、穿刺活检、多种影像学检查和临床随诊。结果:20例患者中,13例(65%)患者最终确定有复发或转移,PET/CT诊断复发转移病例的灵敏度为100.0%,特异性为85.7%、准确性为95.0%。13例患者病理或随访确诊32处病灶,PET/CT共发现29处阳性病灶。PET/CT诊断复发或转移灶的灵敏度、特异性、准确率、阳性预测值及阴性预测值分别为84.3%(27/32)、81.8%(9/11)、83.7%(36/43)、93.1%(27/29)和64.2%(9/14)。20例患者中,11例肿瘤标志CA125升高,10例PET/CT检查显示复发或转移病灶,并经最终结果证实,1例血清CA125水平升高但PET/CT未检出转移灶,至随访结束未发现复发或转移。另有3例血清CA125水平正常,但PET/CT检查发现转移灶。结论:18F-FDG PET/CT检查诊断卵巢癌术后复发转移灵敏度、特异性和准确率较高,在卵巢癌术后监测中有重要作用。 OBJECTIVE: To evaluate the clinical value of PET/CT in detecting and diagnosing the recurrence and metastasis of ovarian carcinoma. METHODS: A retrospective study was done on 20 patients with ovarian cancer. The duration of followup was 6--18 months. The diagnosis of recurrence and metastasis was based on pathologic examination, multi modality imaging and clinical follow-up. RESULTS= Thirteen (65 %) of 20 patients had recurrence and metastasis. The sensitivity, specificity and accura- cy of PET/CT in detecting recurrence and metastasis were 100.0%, 85.7% and 95.0%, respectively. Thirty-two lesions were confirmed in 13 patients, and PET/CT imaging found 29 positive lesions. The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of PET/CT in detec- ting recurrence and metastasis of all lesions were 84.3% (27/32), 81.8%(9/11), 83. 7% (36/43), 93. 1%%(27/29) and 64. 2%/00 (9/14)respectively. The serum carbohydrate antigen(CA)125 was increased in 11 patients, 10 were confirmed recurrence of metasta- sis, the other 1 was closely follow up, but no sign of recurrence or metastasis was found. PET/CT detected recurrence and metastasis in 3 patients whose serum CA125 was normal. CONCLUSION: 18 FFDG PET/CT can detect the recurrence and metastasis of ovarian cancer with high sensitivity, specificity and accuracy and it has high value in monitoring and locating the recurrence of metastasis.
