摘要 目的:观察双水平气道正压通气(BiPAP)呼吸机面/鼻罩正压通气治疗急性左心衰所致低氧血症的作用及其对心功能的影响,评估其治疗急性左心衰所致低氧血症的临床疗效。方法:对18例急性左心衰并低氧血症患者,在常规药物抗心衰治疗基础上联用BiPAP呼吸机面/鼻罩正压通气治疗,观察患者治疗过程中动脉血气、呼吸频率(RR)、心率(HR)、血压及临床症状变化,并进行相关分析。结果:18例患者临床症状明显改善。BiPAP呼吸机面/鼻罩正压通气治疗后2h动脉血氧分压(PaO2)较治疗前显著增高(P<0.01),RR及HR较治疗前显著降低(P<0.01或P<0.05)。结论:BiPAP呼吸机正压通气治疗可明显改善急性左心衰患者的低氧血症和心功能,可作为其治疗手段之一。 Objective: To observe the effect of facial mask/nasal mask positive pressure ventilation with BiPAP ventilator on hypoxemia due to acute left heart failure and heart function. Methods: Treated 18 cases with hypoxemia due to acute left heart failure with facial mask/nasal mask positive pressure ventilation by BiPAP ventilator based on routine anti-heart failure drug. Observed arterial blood gas, respiratory rate (RR), heart rate (HR), blood pressure (BP) and clinic symptom, and made correlation analysis. Results: All symptoms of patients were obviously improved. 2h after treatment, arterial partial pressure of oxygen ( PaO2 ) significantly increased ( P 〈 0. 01 ), RR and HR significantly decreased (P 〈0.01 or P 〈 0.05). Conclusion: Facial mask/nasal mask positive pressure ventilation with BiPAP ventilator can markedly improve hypoxemia due to acute left heart failure and heart function, and is reliable choice for treatment.
