老年不稳定性心绞痛98例临床分析

Treatment of unstable angina pectoris in elderly

目的:探讨老年不稳定性心绞痛患者的治疗方法。方法:回顾性分析98例不稳定性心绞痛患者的临床资料,50例采用阿司匹林100mg和氯吡格雷75mg(双抗)+抗凝进行治疗,48例采用单纯阿司匹林100mg(单抗)+抗凝进行治疗,分析并比较双抗+抗凝组和单抗+抗凝组的疗效。结果:双抗+抗凝组好转45例(90%),恶化4例(8%),死亡1例(2%),半年复发率为5%。单抗+抗凝组好转33例(68%),恶化14例(30%),死亡1例(2%),半年复发率为30%。结论:小剂量双抗治疗高龄不稳定性心绞痛患者相对安全,可明显改善其临床症状及预后,双抗治疗疗效优于单抗治疗,合并高血压、糖尿病、感染的患者同时要治疗基础疾病。

Objective： To investigate the treatment of unstable angina pectoris elderly patients. Methods： Clinical data of 98 elderly patients with unstable angina pectoris treated with different regimen were studied retrospectively, including 50 patients in group A treated with aspirin 100 mg and clopidogrel 75 mg and anticoagulant agent, 48 patients in group B treated with aspirin alone 100 mg and anticoagulant agent, the effects were compared. Results： After treatment, the improved rate, worse rate, death rate and recurrence rate of half a year in group A and B were 90% （45/50） ,68% （33/48）, and 8% （4/50）, 30% （14/48）, and 2% （1/50）,2% （1/ 48） ,and 5% ,30% respectively. Conclusion： Treatment of elderly patients with unstable angina using low -dose double anticoagulant agents has better effect than single anticoagulant agent, which is relatively safe and can significantly improve the clinical symptoms and prognosis ; for the patients with hypertension, diabetes and infection, simultaneous treatment of the original diseases is required.
