关节镜下应用同种异体肌腱一期重建前后交叉韧带 被引量:2

One-stage arthroscopic reconstruction of anterior cruciate ligament and posterior cruciate ligament after knee joint dislocation using fresh-frozen nonirradiated allografts

摘要 目的:探讨膝关节脱位后应用同种异体肌腱一期重建前后交叉韧带的治疗效果,并分析可能影响疗效的因素。方法:2006年8月~2008年5月,应用关节镜下同种异体肌腱一期重建膝关节脱位后同时断裂的前后交叉韧带11例,男9例,女2例;年龄19~57岁,平均38.5岁。右膝7例,左膝4例。结果:11例均获得随访,平均13.6个月(7~27个月),应用改良Lysholm评分标准评价疗效,优8例,良3例,可0例。膝关节功能Lysholm术前评分平均为(49.3±3.5)分,术后膝关节功能Lysholm评分(96.8±3.3)分,与术前相比,差异有统计学意义(P<0.05)。术后膝关节活动范围(121.3±8.6)°,与与术前(83.6±9.3)°相比,差异有统计学意义(P<0.05)。结论:关节镜下应用同种异体肌腱一期重建膝关节脱位后前后交叉韧带是一种确切的治疗方法。 Objective:To evaluate the clinical result of one-stage arthroscopic reconstruction of anterior cruciate ligament and posterior cruciate ligament after knee joint dislocation using fresh-frozen nonirradiated allografts.Methods:From Auguest 2006 to May 2008,11 patients were treated by one-stage arthroscopic reconstruction of anterior cruciate ligament and posterior cruciate ligament after knee joint dislocation using fresh-frozen nonirradiated allografts.There were 9 males and 2 females,and the average age was 38.5 years (19 to 57 years).The lesions were right knee in 7 and left knee in 4.Results:All the patients were followed up from 7 to 27 months (average 13.6 months).The patients were evaluated according to Lysholm knee score and the modified Lysholm knee score. The functional result of knee joint were classified as excellent in 8,good in 3,fair in 0,and the average score before operation was 49.3±3.5,post operation was 96.8±3.3,the different was of significance (P〈0.05). The movement ranges of the knee were (83.6±9.3)° preoperatively and (121.3±8.6)° postoperatively (P〈0.05). Conclusion:One-stage arthroscopic reconstruction of anterior cruciate ligament and posterior cruciate ligament after knee joint dislocation using fresh-frozen nonirradiated allografts is an effective method for the treatment of kneejoint dislocation.
