早期应用柴芍承气汤治疗急性重症胰腺炎的临床研究

The Clinical Research of Using Chai Shaochengqi Soup to Treat the Early Time Acute Critically Pancreatitis

目的探讨柴芍承气汤治疗SAP的临床疗效。方法53例SAP患者随机分成对照组26例和观察组27例,对照组给予常规治疗,治疗组在常规治疗的基础上加用柴芍承气汤治疗。观察并比较两组患者症状恢复情况及综合疗效。结果观察组比对照组的腹痛持续时间、胃肠减压时间、肠鸣音减弱或消失时间、平均住院日等明显缩短(P<0.05),但外周血高淀粉酶血症及高脂肪酶血症持续时间两项指标无显著性差异(P>0.05)。两组患者总有效率比较,观察组96.30%明显优于对照组84.62%,差异有显著性(P<0.05)。观察组部分患者出现严重腹泻,每日5～7次,予胃管注入中药量减半,生大黄改用制大黄,并注意补液,维持水电解质、酸碱平衡。两组均未见其他不良反应。结论柴芍承气汤结合常规治疗重症急性胰腺炎能明显缩短病程,减轻临床症状,具有推广应用的价值。

Objective To discuss the clinical curative effect of Chai Shaocheng soup treats SAP. Methods All cases were divides into the control group 26 examples and the observation group 27 examples. The control group were give the conventional treatment, the treatment group adds in the conventional treatment foundation with Chai Shaocheng soup. To observe and compare the two group of patient symptoms to restore the situation and the comprehensive curative effect. Results The observation group compared to control group＇s abdominal pain duration, the stomach and intestines reduced pressure time, the abdominal rumbling sound weakens or evanishment time, the average in hospital date and so on obvious reduction（P〈 0.05 ）. But circumference blood high starch fermentemia and high fat fermentemia duration two target not remarkable differences（P〉 0.05）. Two group of patients treat the comprehensive curative effect comparison,observation group total effectiveness 96.30%, the control group 84.62%, the observation group surpasses the control group obviously,has statistics difference（P 〉 0.05 ）. The observation group partial patients have the serious diarrhea, daily 5-7 times, gives the stomach tube to pour into the dose to halve, lives the Chinese rhubarb to change to the system Chinese rhubarb, and pays attention to the fluid infusion, the maintenance water electrolyte, the acid-base equilibrium. Two groups even have not seen other untoward effects. Conclusion Chai Shaocheng soup union convention treatment the critically acute pancreatitis can reduce the course, and reduce the clinical symptoms obviously, has the promoted application value.
