非酒精性脂肪性肝病患者血清白介素-18、白介素-8、肿瘤坏死因子-α水平变化的临床意义

Clinical significance of the changes of IL-18, IL-8 and TNF-α levels in serum in patients with NAFLD

目的探讨非酒精性脂肪性肝病患者血清白介素-18、白介素-8、肿瘤坏死因子-α水平变化的临床意义。方法分别用酶联免疫吸附分析(ELISA)法及放射免疫分析(RIA)法检测60例NAFLD患者(NAFLD组),其中非酒精性脂肪肝患者30例(NAFL组),非酒精性脂肪性肝炎患者30例(NASH组)和30例正常对照组血清IL-18、IL-8、TNF-α的水平;常规方法检测ALT、AST、TG、TC的水平。结果NAFLD组、NASH组与对照组比较血清IL-18、IL-8、TNF-ɑ、ALT、AST、TG、TC水平差异具有非常显著性(P<0.01),NAFL组与对照组及NASH组与NAFL组比较,上述指标差异均具有显著性(P<0.05)。结论IL-18、IL-8、TNF-α水平随肝细胞受损程度加重而增加,并与ALT、TG水平呈正相关。IL-18是T细胞介导肝损伤的必须媒介,TNF-α是介导肝损伤的主要因子。血清IL-18、IL-8、TNF-α浓度的高低可用于判断NAFLD患者肝损伤的严重程度及监测疾病变化的指标。

Objective To study the clinical significance of the changes of IL-18, IL-8 and TNF-α levels in serum in patients with NAFLD. Methods ELISA and RIA was used to test the level of IL-18,IL-8 and TNF-α, including 60 cases of patients with NAFLD（30 for NAFL and30 for NASH） and 30 cases of normal controls; the serum levels of ALT,AST,TG and TC was examined with routine method. Results The level of IL-18,IL-8,TNF-α,ALT,AST, TG and TC in the NAFLD and NASH groups was markedly differ from the normal control group （P〈0.01）; NAFL group was markedly differ from the normal control group , NASH group was markedly differ from the NAFL group（P〈 0.05）. Conclusion The level of IL-18 ,IL-8 and TNF-α is positively associated with the degrees of liver damage. The level of IL-18,IL-8 and TNF-α is also positively associated with the level of ALT,TG. IL-18 is an important medium to T-cell mediated liver damage. TNF-α is an important factor of liver damage. Serum IL-18,IL-8 and TNF-α levels may be important clinical index in judging the degree of liver damage and disease development in patients with NAFLD.
