腹腔镜胆囊切除术术中处理冰冻样三角的手术技巧

The operative technique of laparoscopic cholecystectomy for frozen triangle

目的:总结腹腔镜胆囊切除术术中"冰冻样"Calot三角的处理经验。方法:回顾分析我院为21例患者根据其术中胆囊三角疤痕化程度,分离三角结构后行胆囊大部切除或全切除术的临床资料。结果:行腹腔镜胆囊切除术5例,腹腔镜胆囊大部切除术16例,无中转开腹。平均手术时间87min,术中平均出血160ml,术后平均住院5.2d。仅1例术后发生胆漏,3d后自愈。结论:采用适当的手术技巧,可为Calot三角"冰冻样"改变的患者安全完成腹腔镜胆囊切除术。

Objective：To summarize the operative technique of laparoscopic cholecystectomy （LC） for forzen triangle. Methods ：Twenty-one cases were enrolled in our study. According to scarfication of the triangle of Calot ,after the structure of triangle was dissociated, we performed laparoscopic partial cholecystectomy or LC. The clinical data of patients were analyzed retrospecively. Results： There were 5 cases of LC ,and 16 cases of laparoscopic partial cholecystectomy. No cases converted to open surgery. The mean operative time was 87min, mean operative blood loss was 160ml, and meanpostoperative hospotalization was 5.2d. Only one case of bile leakage occurred ,and healed spontaneously on the third day after operation. Conclusions：We conclude that LC for patients with frozed Calot＇s triangle can be performed safely with proper techniques.
