可冲吸电针的临床应用研究

The clinical analysis of the new electric needle which can irrigate and aspirate

目的:探讨传统电针与可冲吸电针在腹腔镜手术中的效果。方法:随机选取2006年10月至2008年9月在本院行腹腔镜筋膜内子宫切除术(如孕8+～13周)的患者58例,30例使用传统电针,28例使用可冲吸电针。腹腔镜卵巢囊肿剥除术(囊肿直径5～12cm)患者70例,使用传统电针和可冲吸电针各35例。比较两大组的手术时间、术中出血量、腹腔冲洗量、麻醉费用、术后发热率等。结果:腹腔镜筋膜内子宫切除组和卵巢囊肿剥除组采用传统电针和可冲吸电针患者的子宫大小、卵巢囊肿大小、术后发热率差异均无统计学意义,使用传统电针和可冲吸电针的手术时间、术中出血量、腹腔冲洗量、麻醉费用的差异均有统计学意义(P<0.01)。结论:使用可冲吸电针在缩短手术时间、减少术中出血量与术中冲洗液使用量方面有明显优势,能明显改善手术野的显露,减少器械多次重复进出操作,缩短手术时间,减少麻醉及术中用药和腹腔冲洗量,节省住院费用。可冲吸电针比传统电针更具先进性、创新性、灵活性及实用性,有广阔的应用前景。

Objective：To investigate the effect of traditional electric needle and new electric needle,which can irrigate and aspirate in laparoscopy. Methods：Fifty-eight patients who underwent laparoscopic intrafascial hysterectomy （uterus as large as being pregnant for 8-13 weeks） in our hospital from Oct. 2006 to Sep. 2008 were randomly selected. They were divided into 2 groups：one was the traditional electric needle group, included 30 cases, and the aspiration electric needle was used in the other group, which included 28 cases. There were a total of 70 cases under laparoscopic remove of ovarian cyst （the diameter of cyst was 5-12cm）. There were 35 cases in each group respectively. Operative time, blood loss, the volume of peritoneal irrigation fluid, anesthesia costs and the rate of postoperative fever between the two groups were compared. Results：There were no difference between the size of uterine and ovarian cysts and the rate of postoperative fever,but the difference of operation time, blood loss, the volume of peritoneal lavage fluid and anesthesia cost was significant （P 〈 0.01 ）. Conclusions ：The aspiration electric needle has obvious advantages in decreasing operative time ,blood loss ,fluid volume for peritoneal lavage and cost of anesthesia. It can improve the exposure of the surgical field, reduce the frequency of equipment coming into and out of abdomen, shorten operation time,reduce the drug used for anesthesia and operation, fluid volume for peritoneal lavage, and cost. The new needle, compared with traditional needle, has seniority, innovation, flexibility, practicability and a bright future.
