摘要 目的探讨组织多普勒成像技术(TDI)评价蒽环类药物对恶性肿瘤患者的左心室功能影响的临床应用价值。方法对80例肿瘤患者应用蒽环类药物化疗前及化疗后6个月进行常规超声心动图及TDI参数比较。结果蒽环类药物化疗后6个月患者舒张早期运动速度(Ve)低于化疗前,(8.68±2.11)cm/s vs(10.52±2.32)cm/s(P<0.01);舒张晚期运动速度(Va)高于化疗前(10.38±1.86)cm/s vs(9.11±1.74)cm/s(P<0.01);Ve/Va低于化疗前,(0.78±0.21)cm/s vs(1.15±0.16)cm/s(P<0.01);蒽环类药物化疗后6个月患者左心室收缩运动速度(Vs)低于化疗前,(7.16±1.15)cm/s vs(8.28±1.25)cm/s(P<0.01)。常规超声心动图反映左心室收缩功能指标:左心室射血分数(LVEF)、左心室短轴缩短率(FS)在蒽环类药物化疗前及化疗后6个月均在正常范围(69.87±5.71)%vs(68.72±6.52)%(、38.24±14.82)%vs(37.75±15.82)%(均P>0.05)。结论TDI可以较早期、更敏感、无创评价蒽环类药物对肿瘤患者左心室功能的损害。 Objective To investigate the tissue Doppler imaging(TDI) evaluation of anthracycline drugs on the left ventricular function of patients with malignant tumor. Methods Conventional echocardiography was conducted on 80 cases of tumor patients with anthracycline chemotherapy before and after chemotherapy for six months and TDI parameters were compared. Results Anthracycline chemotherapy for six months in the patients showed that early diastolic velocity(Ve) was lower than before chemotherapy,(8.68~2.11) cm/s vs (10.52-±2.32) cm/s( P 〈0.01) late diastolic velocity(Va) was higher than that before chemotherapy, (10.38± 1.86) cm/s vs (9.11 ± 1.74) cm/s (P 〈 0.01) ; Ve/Va was lower than before chemotherapy, (0.78 ± 0.21 ) cm/s vs ( 1.15 ± 0.16 ) cm/s ( P 〈 0.01 ) anthracycline chemotherapy for six months in patients showed that left ventricular systolic velocity(Vs) was lower than before chemotherapy, (7.16±1.15) cm/s vs (8.28±1.25) cm/s( P 〈0.01). Conventional echocardiography reflected left ventricular systolic function indicators:left ventricular ejection fraction(LVEF),left ventricular fraction shortening (FS) in the chemotherapy drug anthracycline chemotherapy before and after six months were within the normal range, (69. 87±5. 71)% vs (68.72±6.52) %,(38.24±14.82) % vs (37.75±15.82)%(bothP〉0.05). Conclusion TDI technology has features in earlier, more sensitive, non-invasive assessment of anthracycline drugs on left ventricular function in patients with tumor damage.
