改良鹰嘴截骨重建钛板治疗肱骨髁间骨折

摘要 目的探讨改良鹰嘴截骨优点及应用重建钛板治疗肱骨髁间骨折的疗效。方法采用手术治疗肱骨髁间骨折63例,根据AO分型:C1型12例,C2型43例,C3型8例。采用改良鹰嘴截骨重建钛板内固定手术治疗,术后一周行肘关节主被动功能锻炼。结果所有患者术后伤口是一期愈合。63例均获得随访,随访时间为3~20个月。本组患者均是骨性愈合,未发现骨不连。肘关节功能恢复满意,优良率是80.95(51/63)。结论骨折粉碎程度直接影响预后。早期手术,选用改良尺骨鹰嘴截骨手术入路,坚强内固定,早期功能锻炼是提高疗效的关键因素。 Objective To investigate the therapeutic efficacies on the modified olecraono osteotomy plus reconstructed titanium plate in the treament of humera intercondylar fracture. Methods Among the 63 cases with humeral intercondylar fracture, according to AO calcication, the Cl-type, C2 and C3-type were in 12, 43 adn 8 cases respectively. All cases underwent modified olecranon soteotomy plus internal fication of reconstructed titanium plate. Active and pasive dirigation of elbow joint was performed in one week after operation. Results The wound was first intention after operation in all cases. Follow-up was got in 63 cases. Follow-up time was from 3-20 months (average 6 months). The patient's fracture was union and no non-union was seen. The functional recovery of elbow joint was satifactory and excellent and good rates were in 80.95% (51/63). Conclusion Earlyoperation, selected surgical approach of modified loecranon osteotomy, reliable internal fication and early dirigation are keys to improve therapeutic efficacies.
