阴式与开腹子宫肌瘤剔除术的对比分析

A study comparing transvaginal myomectomy and Abdominal myomectomy

目的比较经阴道子宫肌瘤剔除术与开腹子宫肌瘤剔除术的临床效果。方法2006年6月至2009年2月在广西灵山县妇幼保健院采用经阴道或开腹子宫肌瘤剔除术共162例，其中经阴道子宫肌瘤剔除术87例（阴式组），开腹子宫肌瘤剔除术75例（开腹组），比较两组手术时间、术中出血量、术后镇痛率、术后病率、术后肛门排气时间及住院时间。结果两组手术时间差异无显著性（P〉0．05），术中出血量、术后镇痛率、术后病率、术后肛门排。气时间及住院天数阴式组明显少于开腹组（P〈0．01）。结论阴式子宫肌瘤剔除术与开腹子宫肌瘤剔除术相比，具有创伤小，术中出血少，术后肠功能恢复快，住院日短等优点，值得推广应用。

Objective To compare outcomes between transvaginal myomectomy and Abdominal myomectomy. Methods We analysed the clinical data of 87 patients underwent transvaginal myomectomy and 75 patients underwent abdominal myomeetomy performed between June 2006 and February 2009 in Maternity and Infant Hospital of Lingshan Guangxi. The two group＇s operation time, blood loss, rate of postoperative fever, postoperative morbidity, time of passage of gas by ahus and lenth of stay were compared. Results The operation time in the two groups were not significantly different. But the blood loss, rate of postoperative fever, postoperative morbidity, time of passage of gas by anus and lenth of stay were significantly different in the two groups （P 〈 0. 01 ）. Conclusion Compared with abdominal myomectomy,transvaginal myomectomy has less injury to the tissue,less blood loss and quicken recovery of gastroint estinal function, short time of stay. The Transvaginal myomectomy is good for apptication.
