经阴道彩超早期输卵管妊娠声像特征及其在保守治疗中的价值分析

Diagnostic value of transvaginal color ultrasound in early ectopic pregnancy

目的分析经阴道彩超早期输卵管妊娠的声像特征并探讨其在保守治疗的价值。方法分析经阴道彩超检查早期输卵管妊娠55例声像特点,并于用药前后连续动态检测包块大小及多普勒血流变化。结果30例附件区胎囊内未见卵黄囊及胚芽,于胎囊周边探及点状血流信号,16例附件区胎囊内可见卵黄囊,未见胚芽,9例附件区胎囊内可见卵黄囊及胚芽,25例探及胚囊周边典型"输卵管环",9例胎囊内可见红蓝相间闪烁的心管搏动,频谱多普勒记录为低阻动脉频谱,6例血HCG增高,行手术治疗,3例妊娠囊破裂行手术治疗。结论经阴道彩超早期输卵管妊娠的声像特征明显,在保守治疗的检测过程中有重要临床价值。

[Objective] To discuss the imaging characteristics and diagnostic value of transvaginal color ultrasound in earlier ectopic pregnancy. [Methods] The imaging characteristics of early ectopic pregnancy were observed in 55 patients with transvaginal color ultrasound before and after taking medicine. [Results] After conservative therapy for 55 patients with methotrexate, the embryos in 46 cases disappeared with a successful treating rate of 83%, among which yolk sac was not seen in 30 cases with spot blood flow signal around embryo sac. Six patients had increased blood HCG and they were treated with surgery, and 3 cases had surgery after rupture of pregnancy capsule. [ Conclusion] Diagnosis of earlier ectopic pregnancy through transvaginal color ultrasound is highly accurate and has important clinical value in the observation of the patients with conservative therapy.
