硫酸异帕米星治疗泌尿系感染107例的临床观察

Clinical Effect of Isepamicin Sulfate on 107 cases of Urinary Tract Infections

目的观察硫酸异帕米星治疗泌尿系感染的疗效和安全性。方法选择2008年05月至2008年12月期间我科在门诊或住院就诊、经尿培养证实的各种泌尿系感染患者107例,其中男67例,女40例,予硫酸异帕米星注射液(依克沙,日本旭化成制药株式会社出品)400 mg,1次/天静脉滴注。记录患者治疗前后的症状、体征变化及血、尿常规、肝肾功能情况,并行中段尿培养。同时记录药物不良反应。结果107例中治愈61例,显效23例,有效21例,无效2例。总有效率为98.1%。总体细菌清除率为96.7%。发生不良反应患者2例,6例次,分别为耳鸣1例次、听力障碍1例次、皮疹1例次、消化道反应1例次,实验室检查发现血清尿素氮一过性升高1例次、血肌酐一过性升高1例次。不良反应总体发生率为1.87%。结论硫酸异帕米星(依克沙)抗菌谱广,抗菌活性强,安全性好,非常适于治疗各类泌尿系感染。

Objective To investigate the clinical effect and safety of isepamicin sulfate in patients of urinary tract infections （UTIs）. Methods 107 cases of urinary tract infection patients were enrolled from outpatient unit and inpatient unit of our departments during May. 2008 and December. 2008. All of the patients were diagnosed as UTIs according to uninne culture and bacterial identification. After enrollment, they were administered isepamicin sulfate injection 400 mg ＊ qd intravenously guttae. The symptoms and sign were observed pre- and post-therapy, other laboratory index included blood routine, urine routine, blood urine nitrogen and serum creatinine, liver function, and medistream urine culture was performed before and post drug administration. All the side and toxic effects were recorded in detail. Results In 107 cases of UTIs, 61 were cured, 23 were excellently improved, 21 were partially improved and only 2 cases were inefficient. The total percentage of efficiency was 98.1%. The successful bacterial clearance ratio was 96.7%. Side effects happened in 2 patients including 6 episodes： 1 episode of tinnitus （0.93%）, 1 episode of hearing disorder （0.93%）, 1 episode of rash （0. 93% ）, 1 episode of gastrointestinal disorder （0.93%）, and 1 episode of temporary elevated blood urine nitrogen and serum ereatinine levels （0.93%）. Total incidence of side effects was 1.87%. Conclusion Isepamicin sulfate has wide antimicrobial spectrum and powerful antimicrobial activity, the safeness is also reliable. It is recommended that isepamicin sulfate is quite suitable for therapy of kinds of urinary tract infections.
