二维超声心动图诊断室壁瘤的临床价值探讨

Clinical value of two-dimensional echocardiography in the diagnosis of left ventricular aneurysms

目的通过对３８例心肌梗塞并室壁瘤患者的二维超声心动图资料进行分析，探讨二维超声心动图对室壁瘤的诊断价值。方法应用日本东芝ＳＳＨ－６５Ａ彩色多普勒超声心动图仪，探头频率２．５ＭＨｚ，常规探查心脏各切面，仔细观察心脏形态、梗塞区心肌回声特点（厚度、强度、运动状况）及有无膨出和心腔内有无异常回声（附壁血栓），并测量心腔大小及心功能。结果二维超声心动图诊断的３８例室壁瘤患者均表现左室某一局部心室壁向外膨出，并持续整个心动周期，膨出的室壁变薄，回声增强，呈矛盾运动。其中合并左室扩大者３１／３８（８０％）、左室收缩功能减低者３２／３８（８４％）、舒张功能减低者２４／３８（６３％）、附壁血栓者１９／３８（５０％）。结论二维超声心动图是诊断室壁瘤简便而可靠的无创性检查，其对室壁瘤诊断有较肯定的临床价值。

おbjectives Analysis the manifestations of Two-dimensional echocardiography (2-DE) of 38 cases with aneurysm and discussed the value of 2-DE in diagnosis of aneurysm.Methods Applying TOSHIBA SSH-65A color doppler ultrasound monitors.They were observed shape of the heart.features of the myocardial echo,abnormal echo and measured the size of chamber and Cardiac function.Results The 38 cases with aneurysm which were diagnosed by 2-DE were all showed bulge of the regional area of left ventricular wall which continued the whole cardiac period.Among the 38 cases with aneurysm,31 cases(80%) were found at left ventricular dialatation;32 cases (84%) at left ventricular systolic dysfundtion;24 cases (63%) at diastolic dysfunction;19 cases (50%) at cardiac thrombasis.Conclusions 2-DE was simple,reliable and noninvasive method in diagnosis aneurysm and 2-DE has affirmative clinical values for diagnosis aneurysm
