经皮穿刺治疗腰椎间盘突出症(附74例分析)

An Evaluation of Percutaneous Aspiration Treatment of Intervertebral Disc Protrusion(Analysis of 74 Cases)

７４例腰椎间盘突出症采用经皮穿刺腰椎间盘电动旋转切除术，所有病例均经ＣＴ证实，显效７４．１％，有效１８．９％，无效７．０％。行经皮腰椎间盘摘除术时应注意：（１）根据ＣＴ片确定穿刺点及进针角度；（２）实际操作时进针角度宁小勿大；（３）在切除椎间盘前半部后，导管宜向后外侧退至椎体边缘，导管尾部向上抬高，尽量切吸突出的椎间盘组织；

74 cases of intervertebral disc protrusion were treated by percutaneous method with electrical driving spinal extractor.The final diagnosis of all the patients were confirmed by CT.The results of 74.1% cases were satisfactory,18.9% of the series showed improvements,whereas 7.0% had no effect.For securing a better result,the method should be carried out with care and the following notice must be observed.(1)The puncture point(C) and the angle of puncture(α) should be calculated according to CT.(2)In practice the angle of puncture should be smaller,rather than bigger.(3)After resecting the anterior part of the intervertebral disc,the trocar should be withdrawn posteriolaterally until the lateral aspect of the vertebral body,then elevate the end of the trocar aspirate and protruded intervertebral tissue as much as possible.(4)Provention of complication should be stressed.
